Feed and Fertilizer Renewals

Instructions

• Check the appropriate box under Brand Status, indicating “Renew” or “Do Not Renew”.
• If all brands listed are being renewed, the fees have been calculated in the left column of the Registration Worksheet.

• If not renewing all brands, use the right column of the Registration Worksheet.
  • Feed:
    • Registration Fee: $2.00 for all products, regardless of package weight.
    • Inspection Fee: $25.00 for products available in packages of 10lbs or less.
  • Fertilizer:
    • Registration Fee: $5.00 for all products, regardless of package weight.
    • Inspection Fee: $10.00 for products available in packages of 5lbs or less.

• Labels are required ONLY if “Rev” (revisions) is indicated on the renewal or if labels have been changed. Write the Product Number in the upper right hand corner of labels.

• Completed package will include:
  • Original Renewal Application.
  • Labels, only if required.
  • Fees.

Companies that want to register new products are required to fill out a New Product Registration form. New products that are written in on the Renewal Registration will not be accepted.
